Cancellation Policy

Payment and Refund Policy Our fees cover professional evaluation, treatment planning, and ongoing clinical support. Payments for completed consultations and services already provided are nonrefundable, except where required by law. Payment does not guarantee a prescription, a particular medication, insurance approval, medication availability, or a specific weight-loss result. A completed consultation remains nonrefundable if the provider determines that medication is not appropriate. Monthly Care Monthly fees cover the services and access described in your enrollment agreement. Once a monthly service period begins, the fee is generally nonrefundable, and unused included visits do not qualify for a partial refund. Exceptions include circumstances required by law or when the practice cannot provide the agreed services. You may cancel future monthly care by contacting care@activelifestyleaesthetics.com before your next billing date. Cancellation stops future renewals and does not retroactively refund services already provided. We will confirm your cancellation and its effective date. Medication and Laboratory Charges Medication and outside laboratory fees are paid directly to those providers. Their refund and cancellation policies apply, and our practice cannot issue refunds for charges collected by them. Exceptions and Billing Questions Duplicate payments, billing errors, and prepaid services canceled by the practice without being provided will be reviewed and refunded as appropriate.
